PHP, MySQL user logins and passwords

I have a client who wants to create a form on their site that allows users to create basically a user account....

Name
email
address
phone
company

they want to use the name and email address as the UserName and Password.

I have a good idea on how to store the info into the Mysql dbase, but how would i set up a login page to query their information against the dbase to allow access into the site?
