Need PHP Form Designed

non-database submit form (submits to email)

In need of a submit form for a custom product website. A site visitor uses the form to pick the items they want, delivery information, and additional notes. When the form is completed, the results are sent to an email address, they don't need to be stored on the server.

Turnaround has to be less than one week. Experience with submit forms (php or otherwise) is a must, samples of past work done on submit forms are a plus. Once I provide full details, you provide a quote for consideration.
