Yes, if the client has a tendency to not answer all questions in an e-mail, a numbered list has seemed to be effective.

The Assignment Questionnaire at Creative Business [creativebusiness.com] I have found useful in the past to get a feel for what the client is all about. Although it is fairly general and not web or seo specific it does provide a good starting point for modification.
