Limiting access by country

Whats the best way to block users from a specific country. I realize no solution is going to be complete, and that there are always proxy situations that will be impossible to prevent. But I want to limit access based on Geography, is there a service or a server side library that will do that for me?

You could try to block a specific IP range [google.com] for the country in question.

So that slightly begs the question. Where can one find a list of the IP addresses assigned to a specific country. For instance, if I wanted to block access from all IP addresses in China, where would In find that list.
