Could someone please tell me if the W-9 form needs to be supplied in every case or only after the threshold of $600 is met?
I've tested a company, did not like the results, made $50 and canceled the service. Now they want W-9 before they send me the check but I do not like to fill out unnecessary forms with my SSN...:)

I'm guessing if you did work for them, they owe you the $$...but probably depends what was in the contract. Chances are you won't get paid until you provide them with the w-9, because they want to be able to write off against their earnings.
