Need to create a user fill out form

any suggestions?

I want to setup a form for where a user can fill out generalized fields (I.E. Name, address, etc.) and I would also be able to add check boxes for certain information that they would be interested in receiving. I've looked all over hot scripts, but I cannot seem to weed through them in finding a simple customizable form that can do something like this. Any suggestions?

Simple. I am not a programmer, I do not know how. I am just looking for a script that I can customize and have the information either posted in a database or sent to me via e-mail when the form is filled out. Know if any scripts that can do this?

Did you come across a script named Phorm? It's a free PHP and easy to install as far as those things go:)

My hosting company provides a cgi script that I can reference and it will process the form for me.
