Recall "script"

need a quick fix

I vendor just announced a recall on a product we sell, but the announcement hasn't been made "offical" yet. We are being aggressive and need to set up a form/script to help manage the recall. Has anyone had to do this in a very quick turn around?

can you expound a bit? what will you need the form to do specifically?

Yeah, sorry about that. Basically capture demographic info and some specific questions, then take that data, import it to excel and/or word for use later. Trying to automate this process as much as possible and get it done as fast as possible too.
