Looking for simple form script

Been having a really hard time with my hosting company - I have a form that uses a script where basically the results are emailed to me and the user lands on a set URL. Very simple, but it keeps failing every few weeks, I have no idea why.

Is there a script (php maybe) that I can use where I do not have to rely on my hosting company, that I can simply enter into the html?

Google tools4php. I have been using this for a while with no problems.

Use google with PHP scripts in your search, and look for the following; they are all form-to-email scripts, some simpler to use than others, but all of them end up doing the same thing: sending the form to your email. I am currently using GC-Formmail php, as it was pretty easy to set up, and it works. They are all free as well.
