I've been experimenting recently with my own self-built contextual ad system.

It's quite simple - I started with a geo-targetting database (just to country accuracy, not state/town etc). That gives me the viewers country. I have then have a script that runs against my own DB operating via a very simple word count script which gives me the basic gist of the topic of the page based on word count/stop words.

My list of affilliate advertisers are stored in a third database table and tagged with specific keywords, so, for example, if a US user is viewing a page about US specific shiny widgets, they get one of my US targetted shiny widgets affilliate ads.

Very simple, was built in a day and is so far proving quite succesful (certainly generating far more revenue than AdSense although I should qualify that for you by saying this is a high paying niche).

I'll be posting more on this once my scripts have been honed slightly. I'm finding the word tagging to be quite critical, and ideally I'm looking for a way to automate that side better.

It started life as a simple geo-targetting system. If you join up with an affilliate program like Commission Junction and manually set ads in a header script geo-targetting to the specific country for that offer, it still works pretty well.
