Competition software

Hello,
I am trying to find a program (CGI?) that will run a competition on my site, but I am having a lot of trouble finding one. I have looked in various CGI script directories but there's not much there.

All I want it to do is collect 4-5 fields of information (eg. name, email, phone number), screen out duplicate entries, randomly select a winner when I ask it to, and be able to export the email addresses to a text file.

Does anyone know of any good programs that can do this? Or reputable people who could write such a program for me?

Groundbreak.com has one. Does quite a few things. Easy to set up, too.
