Place 'review' on site

Hi all,

Can anyone tell me how to place a item on my site where users can review something. I would be wanting text like 'Review this item' and then take them where it can be marked from 0-10. Is there script or software out there?

Have you checked out the script repositories such as hotscripts [hotscripts.com] or sourceforge [sourceforge.net]? Something may show up there.
