i think this belongs to the needs, could be adobe pagemill or dreamweaver mx or even a text-editor.

Depends on your budget. If your looking for a free program Amaya is something you want to check out.

This is a question that is hard to answer, since it really depends on the user and where they are in the learning curve, also factor in are you more design or more code or do you need both? Like jbinbpt mentions, it also depends on your budget, since some packages run quite a few $$$.

That said, my current tools of choice are Dreamweaver MX for WYSIWYG, HomeSite for code edits, although Notepad still comes in handy, and on the server Vi or Pico...
