1. How effective are they?
2. Which is better?
3. Are multipage doorway domains a good idea?
4. If I use doorway domains should they be on a different server then the primary website?
In regards to the last question I have 3 web servers
1 in California
1 in Ft. Lauderdale
1 in my office
All out Internet sites are run off the FL. server
Our Intranet/Extranets are run from our office and the server in Cali.
Should I have the doorway domain reside on the Cali and Office server? Or can I also host these domain on our FL server? Reason I ask is the ips on the FL server are all in very close range ie xxx.xxx.xxx.152, xxx.xxx.xxx.153, xxx.xxx.xxx.154 etc... and I dont want that to hurt our rankings if they find out its the same server. Whould that be overly cautious?
Sorry if this doesnt make sense =¦

Doorway pages and doorway domains are a big no-no at most search engines, especially Google. Read any of the webmaster's rules and regulations at most SE's and you will soon recognize that they are a very bad idea.
Where I am, we NEVER would attempt such a thing. Doorway pages were tolerated in the old days on the Internet. But they were abused so much that most SE's don't allow them anymore.
When a person is serious about his or her web site, that person should always show the same content to either spiders or real humans.
Create great content on your site and Google will rank it high. Also, a much better use of time would be to go on a link exchange program to increase your PR.
That's my view on it and I know I speak for most of us here.
:-) Good luck
