Creating favicon.ico... how?

What icon size? What program to use on Linux?

I want to create a favicon.ico file for my website. However, I have a couple of newbie questions about it.

1) What size is the icon?

2) What program can I use for creating the favicon.ico? Do I need a special program or would any graphics program do? If I need a special program, what Linux programs are there for creating the icon?

I guess these are very basic questions, but thanks in advance anyway.
