The Client or the Host?

Which comes first?

Is it the client's responsibility to chose a web host? (I'm assuming that's a 'no,' but I want to make sure.) And if it is the web designer's, should s/he have an account already or choose one during the designing? Who makes sure that the host is paid every year? Is it all on client by client basis?

Are you doing independant design for your own clients? Or are you working for a small company expanding into web development?

Either way I would suggest you do all the dealing with the host yourself. So include the cost of finding and purchasing hosting in your overall contract.

And do it now - the sooner you become familiar with this process the better.

Also I found it very useful to own a reseller package where you own a 'chunk' of server space that can hold multiple domains - so you can then charge future customers the same value for a service you own already. It may seem like a con - but I think it is good business - in any case they are paying the same as your initial customer anyway.

If a client isn't already set on a host, they will likely be happy to let you handle this. They will trust your professional judgment to place them with a host suitable to their needs. You get the benefit of placing them with a host that you are both comfortable with and familiar with. There's good profit in selecting the host. Their costs don't move much from what they would have been anyway, but you pocket almost all the cash.
