Require once for Linux/Fedora

Hi all,

I wonder if someone might assist me:
I am not able to connect my script INSERT INTO etc.
to the database mySql on a Linux/Fedora Server via the command "Require once/ Connection".

Maybe the action:"editForm (PHP-SELF) doesn't match
for Linux/Fedora?

Would be glad if someone could answer. Thank you in advance
