need a template to output data

I need to creat a template that can output date supplied to a Mysql DB. The user can upload pictures etc etc (sort of like myspace.com)

That's a very broad question. Could you specify a little more detail? Are you looking for a designer to create a page design for you? Or an experienced CSS designer who will code an existing design into a page with sql database query strings, or are you looking to add a template you already have to a page that queries a sql database but need the server scripting to help make it all happen?
