Does anybody know how broad matching works?

For a phrase 'X Y', how are the results made up? Are sites bidding on the exact phrase 'X Y' guaranteed to be listed above broad matches? What governs the positions made up from the mix of broad matching of 'X' & 'Y'?

Is there any documentation about broad matching anywhere?

No, it's based on adrank. However, the theory is that "exact matches" generally have higher CTR's (and therefore higher adrank) because of their precise targetting.
