What are the factors that make your Adwords ads appear up top?

Without wishing to go over it all again, can someone explain?

Trying to work out how to get our ad into one of the empty slots up the top for Adwords, as we are currently at the top on the right.

I think its the adwords being in Premium slot Ben is more interested in rather than the CPM v CPC changes.
