Just saw an adwords ad with an opt-in form. Does anyone know if this is in beta--if so when it may be released--or if it's a premium advertiser option?

These are part of the contact beta. There's no word on the many ad extension betas of when they might be released publicly (or if they ever will).
Overall, the contact beta hasn't done that well for most people I talk to (and for the accounts that I've used it in).
If you've got a rep, you can ask to try it out for yourself.
