My campaign's ads go to 2 different domains. Can I set up Sitelinks, or would I need to separate the 2 domains into different campaigns?

Question is not clear: Sitelinks can be used in both options.

It looks like Sitelinks (and all other ad extensions) can only be defined at the campaign level, so if I use Sitelinks in a campaign which has ads going to 2 different domains, the Sitelinks will point to the wrong domain some of the time.

You are right. Currently they are only campaign level sitelinks. The only way you could get them working the way you want would be to create a new campaign for the other domain sitelinks.
