Has anyone experienced problems with adverts not pausing? I had a recent campaign and Paused an advert due to a site change. I saw the paused campaign. When the new site was live I logged in two days later and found the advert had been running and cost £400 per day for 2 days?

I've worked with the AdWords team for quite some time - more than two years - and in that time have not personally seen a case in which a paused Ad Group re-started itself. I'm not saying it's impossible, just that in all my time with AdWords I've not seen it - nor heard of a documented case.
I think the best thing to do is to contact AdWords support, who have access to a complete history of all changes to each of your campaigns, which are noted as to source, time, and date. With this information, they should be able to get to the bottom of things in short order.
