What are the factors?

Sometimes on Google if you search for widgets - two sites will be listed at the top of the page and the rest to the right.

Then other times only one will appear for the same keyword. What factors determine this?

This also happens on Yahoo and MSN. Sometime there will be 4 listing on the top and other times just two.

Does anyone know why?

The top listings are called 'premium positions'.
