Have skied the budget per day and maximum cpc (across all bids within the campaign), and sometimes rank at about position 4, but most of the time the ad is still not showing.

If your percentage for one ad within the ad group is low relative to the other ads, it is because it isn't performing as well. It is an indication that you might want to edit or discontinue the ad.
Now that I understand that the "Ad Served %" relates to ads within the group I realise I am looking in the wrong place.
I have not yet had enough traffic on these ads to work out which ads to use anyway.
The problem is that sometimes I search and the ad is there, sometimes not - so it seems to be a problem with something else (which would usually be budget per day, now skied).

It seems that there may be a lot of factors for one ad not being served as often as others. For instance one ad might match up better with some of the keywords with higher impression/CTR numbers. In that case, it will get more impressions. I think the thing to focus on is whether or not the CTR and CPC are a problem.
