Adsense ad code appeared so many times on sites I don't own

I have observed a new noticed on the dashboard of my google adsense account in which adsense claims
In the past 7 days, your ad code has appeared a significant number of times on sites you don't own. To avoid lost revenue, make sure to add these sites to your verified sites list in Site Management


in the section of MY SITES --> manage site: there is only one (1) approved site (my website) and the other one that uses my ad code is not verified and of course i will not verify it.

My questions are:
-Is this may cause me an adsense account closer?
-How to report such activity to google adsense ?

Please answer my questions and provide me a guide on how to avoid such activity.

You can't avoid it, you don't need to report it, you don't need to do anything. You can read more here in another recent discussion: [webmasterworld.com...]

Just a heads-up - if unknown sites are using your Adsense code, chances are your pages have been scraped and copied to another server, ads and all. This is thievery. You may want to address this.

I had that a long time ago where mine appeared on foreign search engines. Presumably framing a whole page of mine.

AdSense told me not to worry, they already knew.

In theory, all you need to do is make sure that the offending sites are not "verified" sites in your Adsense account (settings > my sites). The UX of that section of Adsense isn't especially well designed. Use the filter option to check for sure that the offending sites are not verified.

In theory, as unverified sites what takes place on them has nothing to do with your account.
The Adsense YouTube chanel has a video about this:

(I assume this spokesperson is pubpolicycomms here on WWW.)

In actuality, I've had this problem and Adsense violations reports were sent to me about actions on unverified sites.
This is the only time I've ever used Adsense support that they didn't follow through and provide me with a final answer. They did state along the way that a bug in their system did exist. I resolved the issue by finally getting the offending site taken down. I don't know if their bug placed my account status in jeopardy or not.
