Widget on other sites landing on a page on my site

I offer a widget that other bloggers/webmasters can place on their sites. This widget is a form, where the user fills some fields and hitting submit will land on a page of my site displaying the results and information about what they entered in the form on the remote site.

I wonder if this is ok to have adsense on the page they land?

The Adsense terms has this item regarding traffic sources:

So my widget could be considered an software application? What do you think?

Are the people coming to your site with the intention of purchasing? Is it worth losing your AdSense account over? Wouldn't be for me. I'd not risk it.
