are they only for fedex club?

Occasionally I stumble upon a site that is using adsense but outside the standard configuration. i.e more than 5 ads per block or lack of the standard "ads by Google" marker. Looking at JS code on these pages it is obvious their using code that is quite different than the ones generated by the adsense site. Anyone can shed some light on this?

They are premium publishers. Last I read, the minimum required to qualify is 20,000,000 monthly page views or 5,000,000 monthly searches.

It's not solely based on pageviews. Some sites are selected by Google to have custom ads. I e-mailed them about it not too long ago, unfortunately there's no way to request to be one of the sites.
