For you unlucky souls without approvals, first of all, i feel really bad for you. Surely, at this point, Google has completely broken its "within 30 days" agreement. If they owed me a significant amount of money, I would be thinking about getting a lawyer to send a certified letter or something to them. Google truly needs to get its act together on the payment process, and at least have some kind of real communication (beyond stupid form letters) to those of you affected. I wish you all luck.

Google changed their terms in February, it no longer states payments will be sent within 30 days, but approx 30 days.

Those without payment approvals, last month they did tell publishers a small number were being delayed.

Also, there can be other reasons for no payments being approved, including accounts without the proper tax information (even if you don't have to submit any tax information for your country, you still have to advise AdSense of that through their tax wizard in the control panel). You could also owe money for Adwords, so log into any Adwords accounts (active or dormant) to check and see if there is a balance owing of older than 30 days. I believe they also hold payments if there is an investigation for fraudulent activity. And sometimes payments get held because they are doing extra checks for a variety of internal reasons.

Contact Google - if you aren't getting a reply, contact them and also supply an alternate email address, incase it is an issue that your ISP is blocking email from Google (yes, it has happened, and to a few people that had been receiving AdSense emails before without an issue.)
