Then this Wednesday they sent the same email, and I'm not serving two Adsense ads on any pages. I run fastclick on some sites and turned off fastclick's google campaigns for all my sites. I think that was what was giving Google a problem.
Now I'm wondering if adsense is getting fouled up when, maybe, a page is served out of google's cache of my site?
Has anyone else run into this situation? (GoogleGuy, do you have an insight into this?)
You might want to send a message to AdSense support and ask if they can give you the URLs of any pages they show as double serving, explaining you believe you corrected the problems causing two sets of ads being shown per page, but you'd like to double check.
Since they have warned you twice, you don't want to find out their policy is "three strikes and you're out" because of accidental double serving on one of your pages. Following up with them would be a good idea.

No, I don't believe that is the problem. Since you have one adsense code and one fastclick code, both these codes are loaded as per advertising settings irrespective of whether they are loaded directly from the site url or from google's cache. Google only caches the site html code, so when you see the google cache version of your site, it still contains your exact fastclick code which would fetch the ads as per settings (i.e. if you have deactivated google from it, it won't fetch it)
