Also, I noticed that on days with such problems the number of clicks (and thus revenue) is significantly reduced (by about 25%), which is no surprise for me as people simply can't click the ads.
I think it is worth reporting it to Google. If it is not widespread, it could be a specific combination of factors (eg: lots of custom channels + blocklists at a certain data centre). They are the only ones who could piece that all together and find the cause.
