I've been up for the past 2 nights trying to figure out how to convert a form to the way it looks in html. Is there a way I can recieve submissions the way they look on the site? I need it this way b/c its the applications have to be sent to a bank. Any help would be much appreciated. My host supports perl and php.

You can use any server side scripting language to handle a form and send the data either to an email address or a database. One more thing you may need to take into account is security. If you are dealing with sensitive information you may also want to look into some form of encryption over secure connection.
