Server help please!

I have Win 2003 Server with SQL 2000 and IIS 6. There has been some type of login that keeps popping up on my screen anytime I try to access the websites on our server. When it first showed up, I received a warning about either isass.exe or lsass.exe. I am not sure which one it was. I am not used to looking for this kind of thing. Anyways, I have been doing some research, have downloaded all latest patches and have rebooted the server. I go to log into the server, no problem. When I try to pull up the sites, that is when it comes up with the security login screen. I can't get rid of it. There have been no login requirements set for the sites and I don't know what else to do.

Plus, after rebooting the server, if I type in the information to login, the site comes back with a 404 error. Can someone please help me?
