I'd like to come off as professional so a little help with my letter would be appreciated.

If the page is not removed I contact the host, provide all of the information I have, and attach the email sent to the owner.
Then, about a day later I usually get an email from the site owner asking why I was so harsh as to have their site taken down.
No template necesssary. Just write from the heart.

Keep it neutral, impersonal, even third-party, in tone. Don't threaten to sue; just provide information. If the recipient happens to assume that you've brought a lawyer in on it and might be planning to sue, you can't help that.
