Hi, I need an agreement / contract for the designer who has created a logo and other designs for us that says that by paying him he transfers and/or gives us all rights to the desgins (to use on the site and in other mediums if necessary like tshirts, etc. Any idea where I can find something like that? Thanks!

Using a search engine for "sample design contract" or similar might yield something you can base your agreement on.
It's common to refer to an effort where the purchaser owns all rights as "work for hire". That might be a good phrase to add to your searching as well.
