Graphics usage rights agreement?

Hi, I need an agreement / contract for the designer who has created a logo and other designs for us that says that by paying him he transfers and/or gives us all rights to the desgins (to use on the site and in other mediums if necessary like tshirts, etc. Any idea where I can find something like that? Thanks!

I use allbusiness.com to get sample contracts and then I modify them for my specific use.

Ideally, what you are looking for is a "Work for hire" agreement.
