Boilerplate Sign Off Contract/Agreement

Anyone know where I might find a boilerplate sign off form pertaining to web work?

Doesn't need to be iron fisted, just something that covers all the basics.

Never really needed one in the past but a couple of recent clients have made me rethink my policies and procedures.
