There once was a time any of my sites would be accepted by any affiliate program running CMP but now I keep getting rejected. Is it just me or what?
My sites are 85%+ US 30,000 - 120,000 unique users per month.

Why not try AdSense.. it is not CPM... but you will get good $$$$ if you have a good topic for the site.
Hope this helps.

Its very very hard. The major reason on why the CPM sites are so hard to get into; is the amount of fraud and inflated impressions that so unethical sites' practice. But you can try the following sites:
321chat your user name implies you may be running a forum or chat based site. If thats the case your cpm or cpc based options will be limited to clickxchange, clixgalore, and google adsense. Cpc programs should not be ruled out if you can test the individual ads like you can with clickxchange and clixgalore. They might net you more cpm wise than a low paying cpm offer.
If you have a "quality" in demand site I suggest Fastclick, tribalfusion, and Burst media for it.

I would skip Burst media. Up to 50% of your banner inventory would have to be surendered to them for their own use. Oh, and you do not get paid for that use either. Tribal Fusion is probably the best, but you still need to have a backup. If you do not, you will be forced to show public ads. ValueClick or ClickAgents are also decent alternatives. Fastclick is very picky, and you would still have to serve some sort of nonpaid or public service ads with them.

I'd agree with the others' suggestion to try AdSense instead of other banner ads. I think it will be a service to your readers (who will likely get targeted yet noninvasive ads) and yourself (4 ads showing probably means a higher clickthrough than a single banner ad). You won't get a set total per thousand views, but from what I hear, you can get quite a bit (not sure if it beats traditional CPM advertising, though).
Plus, you'll be potentially showing MY adwords to more people. :D

But if you do not a have a high CTR, then you will suffer with Adwords. That's the biggest drawback.

Stay away from Clixgalore.com. I have spoke to some webmasters, and they had said Clixgalore hasn't paid them for their commissions for months. I think that their might be some problems going on there. First the merchants, now the affiliates. Something screwy is happening, and they are not doing anything about it.
