Reviews with feedback and rating

Looking for code to integrate visitor comments and ratings

I am trying to find something that allows me to publish reviews on an existing site (PHP, MySQL)and collect user feedback in the form of usefulness votes and comments - a little like the book reviews on amazon but not necessarily so sophisticated. Any advice?
