The best way to format URL's

What is the best way to make my URLs:
dir/keyword1/keyword2.html

or dir/full-title-spelled-out-in-the-url.html
?

Either should be fine but I got a feeling that the keyword-keyword-keyword style will be frowned upon eventually by search engines.

Anyway you get the point, ugly URL's dont do anyone favours, unless you add a bookmark link or something, nobody is ever going to really remember what the URL is that way.

Changing the subject ever so slightly; which is better:

www.site.com/dir/keyword_keyword_keyword.html
or
www.site.com/dir/keyword-keyword-keyword.html
or finally
www.site.com/dir/keywordkeywordkeyword.html

I personally don't think you do need .html and I prefer to see urls of site.com/foo/bar/foo/
