What Ad Networks do you use, and in what tiers?

I'm relatively new to the publishing world, and I'd just like to know which Ad Networks you guys use to generate revenue.

For instance, something like:

Fastclick -> Casale -> Tribal Fusion -> Burst Media

It would also be helpful if you could say why you chose your primary provider over the others. (If you don't want to give away which Ad Networks you use, then maybe just a recommendation of a good choice or two would be great.)

That's it. Linkworth are reliable, but don't generate much income (for our line of business).
