What's the going rate to put a small banner ad on a page? Are there typical sizes and typical rates? Also, how is it that some banner ads flash and the text changes? How is that done?

Rates are all over the board depending on the size of the banner, and whether it is a general ROS adv - or for a more targeted audience. For non PPC, figure on $0.50 to $5.00 CPM for non-targeted, or $40-$60 or more for a more focused campaign. Look around in your industry and compare. There are a lot of 'deals' out there if you can find them.

The ones that are Animated are typically made using Flash or Using Ani-Gifs (produced in a host of image editing software)

Casale quote the following rates:

Leaderboard:
US - $1.35 CPM; daily max. 300,000; frequency cap of 1 x 1
UK - $1.20 CPM daily max. 50,000; frequency cap of 1 x 1
EU - $0.80 CPM; daily max. 10,000/country; frequency cap of 1 x 1

My own experience is that the more you have to spend, the lower CPM you can achieve. However, many web sites will not carry banner adverts at a low CPM.
