Bid Request Form

Is there a generic bid request form available?

I have a prospective client who would like some kind of bid request form so they can document what they would like. Since they are not at all sure of what they would need it's their hope that the form would help. They are non-profit health related with low budget and said that they would take any kind of bid request form to get them started.

have you looked at just general form mailer type scripts available from sites like [hotscripts.com...]

That would be a good place to start, there should be something usable.

I guess my post wasn't too clear. They just want a paper form that they can fill in with their own requirements so that I and my area competition can bid on doing their website. It's required by their non-profit and grant funding status. Their hope is that it would give them hints as to how to formalize their needs.
