Creating an editable PDF with text fields

I have an application that I want to convert to a PDF so my participants can fill it out online, save it and print it. I need to include text boxes and check boxes that they can edit on the PDF. Does anyone know the best process to do this? When I create the application in Word and then print to Acrobat Distiller the text boxes are just images and not editable. Thanks for any help.

I'm pretty sure you have to add this feature to the PDF file via Acrobat. I haven't done this in a while but I believe the Form component will allow you to add editable fields to your document.
