What Do You Charge For Monthly Maintenance?

I am kind of in a dilema here. This is the first customer that I am going to be designing a site for, and I need to know what you charge for monthly maintenance. Do you do just a flat rate a month, or do you just charge when you do maintenance? Please let me know.

I quote a flate annual rate, but I tie it to a maximum number of hours with the understanding that hours in excess are billable. The flat rate is at a discount to the hourly rate.

Using round numbers, annual maintenance fee of $2000 to include up to 40 hours. Hours beyond 40 during the one year maintenance period to be billed at $60 per hour. Minimum of one hour charged per billable event, additional time billed by the quarter hour.

Money in the pocket is worth the discount. Having a one hour minimum helps to dissuade the all too frequent, "Let's change that picture," or, "Let's change the wording just a tad."
