Are Brand Name Registrations Really Necessary Now?

We;re not in the late 90s/00s whereby many companies bought various extensions "just in case".

For sure there is still a lot of garbage out there however do you feel that brand protection is still needed and, if so, why?

Yes, protect the brand. I don't think that will change, imho.
I know of examples where unscrupulous individuals cause disruption, even if they can't retain the protected trademark.
If there's the slightest chance the company might choose to operate in the country, it's worth registering. It shouldn't be necessary for smaller businesses, unless it's in their own country. For example, .com and .us.

Always protect the name. You will have to pick and choose how many TLDs you have to protect, but you can ALWAYS rely on LAWYERS even for those you don't buy up. As long as you are active in protecting the name/mark, you will likely prevail if you have to go the suit route.
