Can someone help me with w8 ben form.

Part1 Number: 6, 7, 8

Part2 Number: 10

Do I have to check Part 3?

About Part 2,Number 9, is it correct to check only 9a(not b, c, d, e)

And is it correct to fill "INDIVIDUAL" in "Capacity in which acting"

You probably need an attorney and/or accountant in your country who is familiar with U.S. tax law to help you.

In particular, I don't know who else could help you with knowing if you qualify for special rates and conditions under a treaty with your country. Several of the questions require this knowledge.

Do not sign as "individual," sign "officer," "president," or "owner," etc. They want your position with the company which should be a position that has the authority to sign off on important documents.
