What kind of source license do I need?

I'm writing a small blog software that I want to publicly release for free.
I want also to give a way the source code, but I want some restrictions.

- Source code can be modified for its own use only.
- Derivative works can not be made using the source code.
- Redistributing modified versions are not allowed.

The thing is, I want to share the code, so whoever needs a modification can feel free to do it so. But I don't want "forks" popping up here and there.
I wonder if there is a kind of license that cover the source code this way.
