Forwarded-For or Forwarded_For?

Other than one renders in the banned file and the other is used in the .htaccess file, is there some criteria that distinguished the differences between them?

Just guessing, but mod_rewrite and SetEnvIf require different formats.

I thought X-Forwarded-For was the header sent, and forwarded_for was the name of the variable in squid to modify that header's behavior or turn it off.
