Can someone please explain and help me understand why when I activate generic joomla htacess.txt file my joomla 2.5.4 rewrites into a /index.php/ category first? This is causing a lot of issues for me as I don't understand the language enough.
What starts out with no htaccess file activated and no joomla friendly urls or rewrite activated files start out as such example.com/index.php?option=com_content&view=article&id=10&catid=11&Itemid=101
By activating friendly rewrite in joomla 2.5.4 I get
example.com/index.php/11-demo-category/10-demo-article
by activating this htacess file and in joomla activating rewrite I get this:
example.com/11-demo-category/10-demo-article
When I want to get this:
examplen.com/demo-category/demo-article
I found a work around dealing with the router.php in the com_content folder where I changed the math as follows:
Removing ID from URL:
change 0 to 1 in in function ContentBuildRoute(&$query)
replace line 27 and 208
and comment out lines 228-232 if statement.
This fixes the issue so now looks like this:
anydomain.com/demo-category/demo-article
Problems arise when I create sub categories or add third party plugins. Does anyone see an issue or is it maybe new joomla and third party bugs? Below is a sample of the .htaccess:Here is the file:

# The line just below this section: 'Options +FollowSymLinks' may cause problems
# with some server configurations. It is required for use of mod_rewrite, but may already
# be set by your server administrator in a way that dissallows changing it in
# your .htaccess file. If using it causes your server to error out, comment it out (add # to
# beginning of line), reload your site in your browser and test your sef url's. If they work,
# it has been set by your server administrator and you do not need it set here.

## Can be commented out if causes errors, see notes above.
