When did this problem start?
What changed just before the problem started? (If you changed nothing, ask your host what they changed.)

The answers to those two questions will give the answer you are seeking.

If you changed nothing, then presumably it has never worked?

Is this a new site/new install?

 Options -MultiViews

may be what you're referring to. This directive should appear above any mod_rewrite code in your .htaccess file, and may be combined with other options, such as

Option +FollowSymLinks -Indexes -MultiViews

In the interest of saving you much time and frustration, let me be completely forthright: I'd suggest that you examine your server config files, referring to *all* of the documentation at Apache.org, and looking up each directive line-by-line as you go to find out what it means -- both technically and practically.

Setting up a server is not a simple thing, and any one of *hundreds* of errors or not-quite-right-settings in your server config files and/or .htaccess files could cause this problem. Invest two weeks or a month in studying the documentation and installation tutorials, and get to where you completely understand the server configuration. Otherwise, the whole project is hopeless -- I mean, we get questions here on very-focused, specific problems from admins at hosting companies, and even though their servers are configured 99.99% correctly, there's really only one single problem, and they fully-understand all the details of server configuration, it still takes a 32-post thread to sort out that one problem. We basically don't have a chance of helping you debug your problem if your understanding of your own server is incomplete.

There is no quick fix to server configuration, and it is certainly not an easy "plug-n-play" proposition. There are hundreds of correct ways to configure a server (depending on precisely what you want to do), and literally hundreds of millions of incorrect ways to do it.

Based on ".htaccess never works," you might want to start with the "AllowOverride" directive, documented in Apache core. There are also several higher-level tutorials at apache.org, describing how to configure various types of servers, and providing example code snippets. We can help you with very-specific details, but one way or the other, you will have to do most of the work yourself, unless you want to spend a year typing here in the hope that someone will answer with a "lucky guess" that happens to be correct...

No offense is intended here, only plain and direct speech. There's just no way to successfully set up a server unless you're well-familiar with what it all means.

Best,
Jim
